Requirements:
- Must be a licensed SLAP or SLP in the state of Arizona.
- Arizona Fingerprint Clearance Card.
- SLPA Rate: $59.00** Base to $90.00 higher tiers.
- SLP Rate: $73.00 Base to $95.00. higher tiers.
- Production Bonus: $250.00.
- $2000.00 Sign-On Bonus.
- $1000.00 Referral Bonuses.
- 5-year bonus: $2000.00.
- 10 Year bonus: $4000.00.
- FREE CEU's offered on a quarterly basis.
- Team Huddles in different parts of the state.

All bonuses are paid out contingent to minimum consistent caseload of 16 treatments per week during the initial sign on bonus, and at least 1 year following the bonus pay out date. All other bonuses are contingent on therapist providing at least 16 treatments per week on a consistent basis.
